Listed below are links to important resources for the US Maritime Academy Captains Class. The Forms section contains links to important forms that you will need to apply and complete your application for a Master Captains 100 ton or OUPV License. There are also links to valuable resources in the Chicago Area.
Forms
You will need the following 4 (four) forms to submit to the Coast Guard with your application. Each will link you to the NMC website for the most up-to-date Merchant Mariner Credential Forms. They are fillable PDF forms that will need to be printed out for final completion.
Merchant Marine Application
Form CG–719B
The primary application.
Medical Evaluation Report
Form CG–719K
Make sure you use this form, NOT the Entry Level Ratings CG-719KE. Download and fill out all personal information including yes/no portions if you are using our physician for physicals. If you are using your own, you will still need to use this form. Print out the form and bring it to your physical exam.
Small Vessel Sea Service Form
Form CG-719S
You can print out and make copies of this form. Once you have created a final draft, you can revisit this link and use the fillable method if you wish. We will discuss this in class.
Periodic Drug Testing Form
Form CG-719P
This form MAY be used to satisfy the requirements for "Periodic Testing Requirements" in accordance with Title 46 CFR 16.220. If you participate in a USCG's "random or pre-employment drug test program," this form may not be necessary.
Gross Tonnage Form
Fillable form and will auto-calculate your vessels GT (PC only)

For hand calculating GT (Use if on Mac or if the other GT Form does not auto calculate)

Use this form if you are renewing your license and do not wish to renew your TWIC card (IT IS NOT NECESSARY to renew TWIC unless you plan to enter secured Ports of entry).
